Use the following form if you have been away from 91²Ö¿âfor more than 3 terms, and your 91²Ö¿âstudent account will be reactivated.
*Please note, if you have received a bachelor’s or master’s degree elsewhere since attending SOU, you will need to complete the Graduate/Postbaccalaureate application with Admissions.
